looks like the device is loading fine...do you mean there is no sound in a recording or the monitored audio is not being heard?

This usually means that the device was powered off when you launched OBS.

Anyway, there are possible solutions:
  1. one that mentions permissions (access rights) for input device in the OS: https://obsproject.com/forum/threads/audio-mixer-issue.181890/post-665025
  2. and other that mentions 3rd party "asio4all" sound driver: https://obsproject.com/forum/threads/video-capture-device-inactive.184250/post-673881

@JoseOBSmodo you likely have bad wiring or malfunction device. At least, these messages in the log:

09:19:51.615: fatal:   No JPEG data found in image
09:19:51.615: Video Capture Device: Error decoding video
says that cable or device or USB port is not OK. Check wiring first.
