My stream keeps connecting for about 5 minutes then drops the connection straight down to 0

Here is my log file from my last test run: https://obsproject.com/logs/HP_ZJESxRq432gsl
I have been trying to figure this out for some time now, basically OBS is not connecting to Twitch. I had this problem before and tried a lot of different things from this forum and even called my ISP got them to fix my line and it worked for a little bit. I'm just wondering if anyone can help me figure out what's going on because I am stumped. Also I use to be able to stream all the time this problem has only been happening in the last month or so.

Per your log
11:10:19.532: Output 'adv_stream': Number of lagged frames due to rendering lag/stalls: 44 (0.2%)
11:10:19.532: Output 'adv_stream': Number of dropped frames due to insufficient bandwidth/connection stalls: 2765 (14.5%)
11:10:19.532: Video stopped, number of skipped frames due to encoding lag: 49/19102 (0.3%)

When it comes to bandwidth, it might be your ISP, or your PC, or your LAN, or between your ISP and the (Twitch) ingest servers(s)
have you followed this thread https://obsproject.com/forum/threads/major-bitrate-issues.139545/

Yeah I did most of the things on there expect changing my Ethernet cable and a couple of other things. but I'm just wondering why it would happen so suddenly. I'm starting to think its my ISP but I don't know I think I just need to keep trying things. also on my twitch test my bandwidth is over 5000 and the quality is 80 so I don't know why its doing it.

LOTS of reasons.... too many to type, many of which have nothing to do with your ISP
buts its late, I'm tired, and it is outside the scope of OBS, so I'll leave it at that for now

though if your upload is only 5000kbps, and you are trying to stream at 4500 isn't leaving room for normal variation. You may be simply shooting yourself in the foot trying to stream at effectively 100% average bandwidth availability. I'd recommend dropping you bitrate down to 4000, or maybe even a little lower and see if that solves your issue
