Bug Report Multiple Issues with Audio Monitoring

Audio Monitoring is a extremely useful feature, but unfortunately doesn't seem to be quite perfect since I've been running into several problems with it. I'll go through them in the order that I encountered them.

The first is that the monitoring audio can end up being desynced after some time. This appears to happen every time I use it. The actual stream/recording audio will be fine, and it can be fixed by simply disabling and re-enabling the monitoring, so it's not the end of the world, but is still problematic since it can cause someone unaware of this to believe there is actually an issue with the audio when there isn't.

The second is the opposite of the above, where the audio monitoring remained in sync while the stream and recordings were desynced. I only had this occur once, but just the ability for this to occur at all damages the utility of this feature as a diagnostic tool, since I don't know if I can actually trust it.

The third and most dire of the issues I've run into this that, starting yesterday, it simply stopped working. Restarting OBS or the computer doesn't appear to do anything, nor has fiddling with the settings yielded any result. The problem is not the headphones, they work perfectly fine with everything else.

Here's a log report, but looking through it I didn't see anything I thought was relevant, asides from the aforementioned unsuccessful setting changes.
I suppose maybe some of the stuff that failed to initialize/load might be it? idk.

I've got an update, and I've also got to hang my head in shame. After a week of trying to figure out what's wrong, fiddling with settings, reinstalling audio drivers, I open up the volume mixer in Windows and see that for some reason, OBS is muted. lmao. welp.

The other issues still stand though.
