Ok, I give up. Nothing I do seems to change this. Every option I try results in one of two things.
I can set the options so I have no echo, but I also have no sound from the game/video I'm recording in that case
I can set the options so I have video/game sound, and mic sound but the mic sound has an echo (feedback?)

Yes, I have thanks. It got me most of the way, but it is unfortunately now a bit out of date for 10.14.2 (Mojave) and because the author has it set for a particular microphone that also acts as a monitor output, it just doesn't quite get it. My set up is quite different.

Echo can caused by one or more of the following:

1) Having the same device captured twice. (Same device selected multiple times in Settings > Audio, or having a device set there and then also added as a separate audio source in a scene

2) Having your Monitor output in Settings > Advanced set to a device you are capturing

3) Having devices in Edit > Advanced Audio Properties set to "monitor and output" instead of "output only" (if you are monitoring through some other method rather than OBS' monitoring feature)
