Disable any GPU overclocks, and completely uninstall and reinstall a fresh copy of your GPU drivers.

This error seems to only ever occur with instability but I unfortunately couldn't say for sure. It forces your driver to "reset" and OBS loses the signal from your graphics processor, so it's a fairly critical failure.
