Low CPU and GPU usage, but getting a lot of rendering and encoding lag anyway

This solves the mystery. Laptops usually running power save mode while on battery. Performance mode (with higher frequencies) enabled when you on power line.

I've already been looking through and messing with the power and battery settings, but I can't figure out why it's switching refresh rates automatically or how to stop it from doing that.

I know only one method that works in most cases - modification of the display driver (Custom Resolution Utility and similar tools). Custom display driver (table of valid video modes) should has only 60Hz refresh rates, so system cannot fall back to other frequencies. In the past it worked OK. User needs some advanced knowledge how to boot from recovery disk, how to install OS and how to connect second display if something goes wrong. If this is not your way - ask manufacturer of the laptop how to fix the refresh rate to 60Hz for your device.
