Question / Help Livestream output is smoother than my gameplay

I'm running a pretty decent rig (i'm sure everyone says this and you're sick of it so i'll skip the specifications, but needless to say the hardware is up to date and up to snuff)

I'm streaming 720p in 60fps. I've had this issue for the longest time (I've been streaming for years) and it really got on my nerves when streaming on an alternate channel of mine. In fact it bugged me so much that I stopped streaming because it made the gameplay unplayable for me.

The stream is fine - the game is being streamed in 60fps, and looks like a solidly smooth 60fps from the stream perspective. But from my perspective, I get loads of lag - what looks like skipped frames, and delayed input possibly.

The game I'm capturing is Freelancer, but this happens in other games, such as Quake Live. (more info below)

My perspective of the lag is a lot worse when I'm not using NVenc, so I stick with nvenc.

About quake live - there is a checkbox to always see your FPS in the corner of the screen. This number is being generated by the game client. if I have my framecap set to 125, it will usually stay at 125fps. (i have a 120hz monitor). When I load up OBS to stream QL, that 125 dips down to various numbers between 110-125, and feels like a solid 50fps when I play. It's an incredible annoyance (sorry if that sounds picky, but at higher levels of gaming, that's a major factor)

Does any of this sound like an issue that has been reported and resolved here before?

Again.. the stream output is smoother than what I see when I play.

also, unticking the Game Capture while streaming (still streaming the rest of the scene list) removes the lag

is this lag generated from just hooking into the client to work its magic grabbing the picture? is there a workaround?

I've had this issue with xsplit when it first came out (and still to this day), and also with OBS.
