You didn't include a log file, so I have nothing to go on, so I'm going to take a look in to my crystal ball and take wild guesses about your setup: you're using some USB 2.0 capture device to capture a console, such as an elgato or LGP, and the device is delayed, and you have to manually delay the mic or webcam, but it didn't work out correctly.
Am I guessing correct? If not, then please definitely post that log file so I can know what's going on.
