Question / Help Issues with Recording Multiple Tracks and Premiere Pro

I have been struggling with this for a while and I want to try and record Multiple Audio Tracks but can't seem to get it to work. I have watched many videos on how to set this up and I'm still very new to this OBS stuff so please bare with me.

Firstly, I have setup to where my Audio tracks setup in this manner.

My Desktop Audio (Headphones) is set to Track 1
My Microphone is set to Track 2
I check to see my mic is working by selecting 'Monitor and Output and I can hear my voice.

Secondly, Under Output in the settings:
Type: Standard
Recording in .mp4 (I read somewhere that .MKV is recommended for multiple audio tracks but Premiere Pro does not support the file)
Audio Track: 1 and 2 are checked
Encoder: H264/AVC Encoder

If there is anything I can post to help with this issue please let me know.

First... don't record directly to .mp4. If you need .mp4 files, then record to .mkv first and remux to .mp4 afterward. You can set OBS to automatically do this in advanced settings (or, use the built-in tool in the file menu).

Second, check your audio tracks in VLC. Multiple audio tracks don't play simultaneously -- under audio, you should see Track 1 and Track 2. See if you have both tracks, switch between them, see if you have audio.

If VLC sees both tracks, then it's an issue with how you're trying to pull the tracks in Premiere. I've never used it, so I'm not that familiar. However, there is this that I found from a quick google.
