Use for 4K preset P4, CQP 20-25. Smaller value - better quality, but larger size.
If size is important to you, then use VBR or CBR

GPU driver crashed about 1.5 hours before you started that 3 minute recording. The studio driver you're using isn't that old (517.40) but you'll get nice performance & stability gains with 522.30. I've been using it for over a month without a single hiccup.

Disable Game DVR & HAGS

Your capture device frame rate & sample rate settings should be updated to match OBS. All of the audio buffering in your log is coming from the a6400 & is being caused by the sample rate mismatch.

If you notice any stuttering in the Preview window or recording when encoding 4k, try setting your monitors to 30FPS w/V-sync on. I was getting very minor stuttering with my 3090 under certain workloads & when I made this change, gone.
