How much bitrate on buffer etc should I put if I have like 40-45MBit upload speed?

Not more than 5000/5000 if You are not partnered with Twitch/Own3d. For 720p@30fps like 3000/3000 is enough.

For better results you could also make sure you're using the lowest ping Twitch server. JTVPing will let you check it easily.

Hell, 2000/2000 is enough for 720p/30fps. But really, once you start going above 3000 bitrate, you start running into people that can't watch your stream, so keep that in mind. Not everywhere has good internet =/

IMO, 2000/2000 isn't enough for fast-motion 720p30. It is probably OK for games like LoL or SC2 though, but not most FPSes. For faster games with more motion, I'd say 2500/2500 for 720p30, or even 3000/3000 if you can.
