Even if I'm in OBS, with nothing else open - hot keys still aren't working. All settings above tried. I'm ona PC with W10 and also using Chrome. This is really frustrating.

OK I found out why it works for some and not for others as I was in the same situation.

You have few things to take into account:

  1. In "'Settings" -> "Advanced", go to the bottom, find "Hotkeys" dropdown and select "Never disable hotkeys".

  2. (Optional) Some games requires you to have OBS lunch as Admin.
So, if it doesn't work, start OBS with Admin rights.

  3. (Situational) If OBS is started as Admin and you window is focus on OBS and you use StreamDeck to activate the shortcut, this might not work if the StreamDeck application is not started as Admin as well.

    So you have 2 options:
    1. Run StreamDeck app as Admin
    2. Disable Admin rights for OBS if you don't need it
I hope that will helps some of you!

I am using 2 keys for Push to Talk and I fixxed my problem by doing the things mentioned before and adding a second Hotkey in the Hotkeys Tab under my Audio Source.

My problem I a little different. My scene switching hotkeys work perfectly but my media hot keys (for sounds and memes) don't. I'm on a windows 10 pc and have never disable hotkeys as well as running in admin mode. I noticed that it tries to start the media source but stops at one second. If I have the media source selected the timer bar will light up blue with 0:00:00 on the timer out of whatever the source time is but never moves further... If I double click the source it runs fine.

On a Mac, you have to give permission for OBS to listen to input devices (keyboard) for hotkeys to work. Go to System Preferences -> Security & Privacy -> Scroll down and select 'Input Monitoring' -> Enable for OBS.app (you may have to click the lock in the bottom left).
And on top of that, go to System Preference->Security & Privacy->Accessibility and add OBS to the apps that are allowed to control your computer. Also, if you have a dead chicken, wave that over the computer three times.

Certain games take over the system resources and disable almost all hotkeys. For example Quake III Arena. Even if it is installed from Steam, the game is pretty much unchanged from the original. It is updated to the latest official point release, and that's it. When I launch Quake 3, having OBS run as Administrator doesn't help. I can't even bring up Task Manager by pressing Ctrl+Shift+Esc, I have to use Ctrl+Alt+Del and click on the Task Manager. That re-enables system resources for hotkeys, you can click on things, click on other windows, but once you click inside the game, it's all over again. If you have Task Manager set to stay on top, then your game is obstructed with Task Manager, but you can't get rid of it, your only hope is again Ctrl+Alt+Del, or just simply close the game by pressing Alt+F4 (yes, that works, too). The issue is the game runs full screen, it actually changes the screen resolution, and you can't really use anything else. Pressing Win key doesn't put you back to Windows opening Start menu, you're limited. I'm not sure if there is even a way to use the hotkeys while in-game if the game actually locks certain system features.
