Your streaming pc is the important one since that is doing the compression. Medium will look better. How much better depends on what game you are streaming and what res/framerate. No one expects streams to be perfect quality. 720p30 on veryfast is too much for many people. You'd be fine on fast, and you don't have to worry about your CPU overheating or whatever.
