Looks like Windows got updated but your GPU drivers didn't.
Perform a clean driver installation for your GPU by following the instructions at http://obsproject.com/forum/resources/performing-a-clean-gpu-driver-installation.65/
and make sure you install the latest driver for your system.

I've checked that all Graphics drivers are Indeed Up to date

The crash is happening in your APU driver, and Windows is very clearly loading one from the filestore instead of the normal place.
Did you do a clean driver install as instructed? If you just used Windows update to check for a newer driver then you failed, must start over and do what was posted. If you just went to AMD.com and downloaded the latest driver and installed it, you failed, must start over and do what was posted.

I've done a complete Graphics driver clean install as you said, and yet still crashes
