Question / Help Choppy Video Recordings and Preview Playback

I just downloaded OBS and have watched several videos on how to set it up and read on these forums how others have rectified this issue. I have had no success. My frame rate is fluid in game but in the preview window and in the recording it's abysmal. I've posted the log below (I think)

I wanted to add what I've changed in an attempt to resolve the issue:

Encoding:
Encoder (tried both x264 and Nvidia NVENC)
QB - 10
Bitrate (from 8000 - 20000)
Buffer size (0 - 1500)

Video:
1080p
30 & 60 FPS (issue better but not resolved at 60FPS)

Advanced:
NVENC Preset (High Quality)
Encode Full Range (checked and unchecked)

I have gotten the issue to be minimal but, the recorded product is grainy and almost sun bleached. The choppiness is more subtle than before but still evident.
