Question / Help Capture Card Options for HD PS4 Streaming

Hi guys, so my goal here is to play PS4 games (Destiny, BF4, etc.) and capture the audio/video of my PS4 and route it live to my PC so I can stream through OBS and take advantage of the settings/overlay/encoding and power of my PC.

I'm familiar with PC streaming, and semi-intelligent regarding pc terms and components (built my rig about 2 months ago so I know my way around a computer, but not an expert). I am not too familiar with the architecture of my PS4 though, so I would need some explanation there as to how all of this would work.

What are my options regarding the right capture card to get, and how would I set it up? I'm sure there are resources available to me on YouTube that I can look into once I decide which card to get, but if anyone knows of a good site or guide that has information that would be very helpful.

As far as the price vs. performance scale, I want to get something nice, that can capture 60FPS games (30FPS in Destiny's case) and have no problems enabling me to stream at 1080p. I don't need a top of the line Swiss Army Knife, as I'm pretty sure I would only use this card for this specific purpose, capturing games from my PS4 that is. But I'm ok with spending $200 if thats what it takes to get something that will do what I need.

Thanks for all the help, let me know if you have any questions

Well, the usual:
Avermedia Game Broadcaster HD (up to 1080p60 input, 30fps recording)
Avermedia Live Gamer HD (up to 1080p60 input, 30fps recording but option to downscale to 720p60)
SC-512N1-L (up to real 1080p60)

USB 2.0 devices can work but will give you sync problems between audio/video/microphone/webcam etc.
USB 3.0 devices can work but need a good USB3 controller and can still have similar sync problems.
There are probably some more devices available for pci-e that will work, and there are of course quite expensive devices available that allow even higher inputs (which your ps4 does not need).

If you're willing to go outside the box a little and have a free PCIe 4x slot (or better) on your motherboard, then you could pick up one of these:

http://www.ebay.com/itm/Datapath-Limited-DGC150C-PCI-E-Video-Capture-Card-VisionRGB-E2-/111380795120

Its a fantastic capture card that can do 1080p60 natively at an outstanding price (albeit used). Risks are those inherent to all Ebay auctions of course. You'll need a HDMI to DVI adapter, but those are cheap.

There's a plugin for Datapath support here (OBS will work without it though): https://obsproject.com/forum/threads/datapath-vision-capture.12503
