Question / Help Black screen with audio working.

First and foremost, the Monitor capture works just fine. That log itself is from a relatively short stream I did whereupon I realized it wasn't working and so began troubleshooting. All the other help I've managed to get online so far hasn't resolved my issue - I either already performed the step, or the specific step they were taking (ex. check the anti-cheat option) was not available in my specific settings.

If the log is too short let me know, I'm going to start running another blackscreen stream as soon as this is over while I play.

I don't really know what the issue is, and this happened to me before. I think when I re-downloaded OBS that fixed it last time, after a reformat. But once again the game capture has gone black on me, and I'd rather not resort to using monitor capture, so what might be the problem?

I selected the uninstall option and made sure to keep searching it through my start bar until I removed two different OBS folders and installations. However, when I re-installed it from the website and I opened it fresh, it had the exact sames scenes and sources as the one I just uninstalled. Does that mean it didn't uninstall right?

P.S. At the top it says 'Profile: Untitled - Scenes: scenes - Open Broadcaster Software v0.651b' and I'm still only getting a black screen when I hit preview or try to stream.
 
Hopefully bumps are allowed, or at the very least, a request for an update. Anybody know what could potentially be the issue with my OBS? Or... computer, or what have you?
 
Is there anybody that might be able to help with the issue? I was kind of hoping I might get some kind of a response that could help work through this. I'm unsure of what I can do since I've already even re-installed the program and re-created the scenes and sources.
