Post a log file from the Help menu. We need that to see what's happening on the back end. Just Upload, then paste the link to it here.

Nothing looks wrong in the logfile; do you have a sample video clip demonstrating the issue?
Only thing I can think of right now is something to do with your Realtek audio drivers, possibly.

Just spitballing, are you running your system audio as more than Stereo? (eg: 5.1, 7.1, etc) I know that OBS can't use more than 2 channels, so it's possible the mixdown to stereo isn't being performed properly if so.

Youtube link I posted is the sample video, it was recorded by me. And as far as I can see, my Realtek driver is set to stereo sound, but the option is locked:

Ah, I'd missed it. Hmm. Possible to do a recording and clap your hands? It's possible that could be what's happening... playback over the speakers, but recording from the mic. Since it's a laptop, I'd assume they're fairly close together. The log says this isn't the case, but I really can't think of why it would sound like it does.

Do you have any reverb effects/spatial expansion turned on, like false-3D sound?

Nope, it doesn't record mic. "Do you have any reverb effects/spatial expansion turned on, like false-3D sound?", you mean in Realtek app or something else? I can't find such option in Realtek so i think i don't have it turned on.
