Question / Help Audio Registering But Not Recording Or Streaming

Hi,

I can see the audio registering because I see the little green line moving as I talk/play music/etc however when I stream or record the video is fine but there is no audio.

Is there anything I can do to fix this? Everything had been working fine for several months with no issues until this. I've already tried reinstalling and restarting several time.

EDIT: I am using OBS 0.16.5 (64bit, windows). also ive added a log file
