Question / Help Audio not in Sync with Video

The audio is running about 4 seconds behind the video and for some reason, I can't seem to find a work around. It happens for both recording and streaming. I've gone through almost all the settings, none of which have impacted it. I've tried a sync offset, but it doesn't take a value any less than -1000 without eliminating all audio completely. I've uninstalled and reinstalled OBS Studio to see if that made a difference, but it didn't. It worked fine 2 streams ago, but hasn't every time before that and now without anything being changed. I'm not sure what to try now because I've tried everything that I can think of and it's quite a hit to stream quality. The following is what shows up in the log:

Try new profile and new scene collection. Build simple scene (color source + mic) and try to click hide/unhide. Make record and playback it. Are you sure that issue still present?

Thank you, I really appreciate the response and help. I created a new profile like you said, added a game capture, and it worked perfectly. Now I just need to figure out what is causing the issue with the existing setup. Any thoughts?

I just did a test recording after hiding, then unhiding all other sources and it appears to be working fine. I'm going to be streaming tonight, so I'll watch the VOD after and see if it actually does work. I would really like to know specifically why this issue arose to begin with though.
