when I choose audio only; you get NO sound period. I had the TV box playing a channel from hdmi capture and was getting sound but out of sync but if I choose the audio only; no sound and that is useless if I tried to stream from my camera, etc there would be no audio
why would someone pic this option?

I can only share my experience based on a ASUS CU4K30 USB HDMI capture device. That option works. However, if your USB capture devices sound is muted / disabled in Windows it wont work.

I get sound with my capture device unless I click the audio only option that someone mentioned earlier, then there is no sound; otherwise I get sound but it is not in sync in OBS but perfectly in sync with VLC, Potplayer and the NDI VLC option. Does anyone know how to stream the video from VLC or Potplayer to youtube for live stream ? on google , all you get is how to stream youtube content to VLC, etc but I am asking for the other way around. I want the VLC or Potplayer Video to be streamed to my youtube channel

Are you aware, that normally, OBS should not play back the sound that it captures?

OBS simply encodes and sends live audio to some specific online service (or video file). This process is called capturing and streaming (or recording). OBS should not playback anything. And only thing you can do - is to view your own stream via the browser, as other users do.

There is special feature of OBS that is called Audio Monitoring it allows to playback specific sources through the specific devices on the same PC, where the capturing task is running.

You really need to read much more info about the capturing and streaming before attempting to do it.

OBS definitely is supposed to play back the audio and video from the HDMI capture card. I have multiple people tell me they use it for exactly that purpose. I am getting the audio and video in OBS but it is not in sync like it is supposed to be. As I wrote earlier, VLC, Potplayer and even the NDI using the VLC option is playing the video with audio in sync. It is ONLY OBS that is failing. I would like to stream to youtube right from one of the other players but I don't think they do that. The streaming from OBS to youtube is very easy exept in this case the audio is out of sync and that means the recording function may also be out of sync if someone wants to record the video from capture device

Try disabling the buffering option in the video capture device settings, my screenshot in my previous post #22 shows the option.

I tried the buffering option but it is still out sync. Trying to play with the audio delay in the audio properties. By disabling the buffering and then playing around with the delays you can make it a little better but it is still on in sync the way VLC and potplayer are
