What happens with me is that my audio lags really far behind my video in game and also my skype gets really out of sync. I have tried using the "Use Audio/Video Sync Fix" option in advanced, but it causes me to disconnect like crazy from streaming servers like once every minute.
My specs are:
Intel i3 2100 Sandy Bridge @ 3.1 GHz
EVGA GTX 560 Ti
6 GB of RAM

my bitrate is 2000, my buffer is 2000, my audio is like 128
idk what I should be setting my stuff to as my interwebs are usually pretty good but it sometimes has this pocket of bad times
