Why the graphic square on Preview?

When using OBS on my Mac, I have a small gray box describing the plugins show on startup. After startup, the same gray box with out text remains on the screen over the preview screen. At first I thought it was the downstream keyer was causing it as it was the title of the plugin that showed on startup However, after deleting that plugin and restarting OBS I still have the blocked out graphic, just a different plugin showing at startup. Screenshot attached. Logfile is at https://obsproject.com/logs/TH1OQqqk05MvAiAD

I also am having the same issue. I see by your log file that you have the audio-monitoring plugin installed. I'm testing mine by going through each of my plugins to see which one is the culprit and so far it seems to be the audio-monitoring plugin causing this issue. If either of you could please either change the name of the plugin by adding another letter to the front of the name or remove it either way and restart OBS to see if this removes the gray box off your screen to help confirm my suspicion, that would be helpful.
