Question / Help Video source: audio, but no video playing

When I cue an mp4 to play as a source in one of my scenes (transitioning over from the camera), I only get a black screen. On the replay of the recording, I can hear the audio loud and clear, but no video plays. I'm running 0.14.2 OBS Studio.

Am I doing something wrong?

Maybe video is out of the screen? Fit it to screen. You may try to disable hw decoding by unchecking Use hardware decoding when available option. Also, please post a log when issue is present (make few transitions while recording or streaming).
