Why is the text you put in your post not contained in the log file you linked?

Are you trying to use an Osprey device or a Datapath device?

Which Windows update broke it?

If the problem is with the Datapath, have you checked their web site for updated drivers and firmware?

Checked for drivers already.
Datapath.

Capture cards behave as webcams when dealing with OBS, so yes you need to check those privacy settings.

There is no privacy setting for the Datapath card.
Card works with FFMpeg, Classic Obs Studio, Ovo video recorder.

I am having the same issue, same symptom, same log. My problem did not begin with a Windows update. My Datapath card is a VisionAV-HD. I am using the most recent Datapath driver released yesterday (7.17.14), and most recent OBS (21.1.2). It has the error "DShow: Could not set video format (0x80004005): Unspecified error" in the log when attempting to initialize this display device. Input works in OBS Classic and using Datapath's own Vision Window.

You both have the april creator's update.
Can you both post screenshots of your webcam privacy settings screens?

It has nothing to do with Windows.
It is also in Windows 7.
Datapath added some colorspaces at some point. After this update it got broken in OBS and still is.
ffmpeg, xsplit and any other software has no problems at all. I wont downgrade that far just because OBS Studio is broken in this regard xD
