Question / Help Video Capture fails intermittently

Using Roland V-4EX as Video Capture Device. 80% of the time there is no issue - but every so often when I go to live stream - there is no video. If I add another Video Capture device and use the same source - the video is there and system streams OK.
seems to happen at the worst possible times! Any Thought are greatly appreciated.
Using latest version of OBS and drivers/firmware on the V-4EX and 64 bit Win 10 system - nothing else running on the Computer.

I have to admit I don't understand the contents of the log file but am attaching a couple. OBS crashed 45 minutes into our second service Christmas eve too. That was a first - but I am also attaching that crash log file. The 12-22 log file is example where the video from V-4ex is blank. I did find that if I turn off the V-4ex and let it reboot - then I can immediately add in the new video capture device and it recovers the video source. The 12-25 log file would show a normal startup. Thank you for you help!
