Unexpected quit / adding sources

Hi, I'm new to OBS but work with LPX and FCPX so I know a little bit about getting things to work.

I've recently installed OBS for Mac silicon and every time I close it says that it quite unexpectedly.

Then my video source no longer works, AND I cannot add a new source.

I've had to completely uninstall the app using appcleaner then reinstall OBS and it opens the same project I was working on (where is that by the way).

Then maybe I can add a new source and replace the video source I already made that stopped working because of the unexpected quit.

Thoughts and solutions please?

I recently updated OBS now I am unable to add new sources. The ( +) for adding sources is doing nothing and tried to click on sources box to add but got same results. Everything else is working fine, just can't add new sources. can delete them but not add, trying to find the solution.
