The only fix I know of is to shut the stream down and restart OBS.

Example of problem: https://www.twitch.tv/videos/455717548

This seems to only occur when I'm switching scenes on my stream PC (in OBS Studio) or when I alt tab a bunch on my gaming PC.

About my setup:
2 PC setup.
Avermedia GamerHD capturecard
Gaming rig specs: 6950x, 1080ti, 32gb quad channel, soundblaster audigy fx (it occurs even if i use onboard)
Stream rig specs: 5960x, 1060 6gb, 16gb quad channel, soundblaster audigy fx (it occurs even if i use onboard)

Both using 64bit windows.

It occurs even if I switch the video cards and use the 6950x as the stream PC. I've only been using the 6950x for a couple months. This has been occurring for years, but it seems I may have narrowed it down to alt-tabbing on the gaming PC as the initial cause and/or switching scenes too often in OBS studio on the stream PC (still not sure).

Things I've tried:
Setting all audio inputs/outputs (aside from mics) to 48,000 / cd quality
Lowering OBS video & stream quality settings (ultrafast/superfast, <4000kb video bitrate)
Upgrading to gigabit internet
Swapping computers (the ones above. I've also had the same issue using an alienware laptop with the 5960x as the stream pc).
Bought sound cards for both PCs.
Routing the audio via HDMI instead of 3.5mm cables.

I can select the audio input from my sounds and select "listen to device" and it sounds crystal clear even while this is happening.

Included a log file.
