Strange issue on just ONE game

Hi everyone. I've really tried so many things so thought I'd post on here to see if anyone else has any other suggestions. I've been trying to stream Skyrim modded (I'm getting these issues with Vanilla too however) I have a 2600x Ryzen, 2060 Super, 16gb ram etc. I stream Warzone, Doom Eternal on Ultra, Red Dead 2 etc. All absolutely smoothly no issues whatsoever, 1080p 60fps, no frame drops. However Skyrim, whenever I load into a new area, indoors or outdoors, it drops like crazy around 200-500 frames across a few seconds. My stream goes from putting out 5k/5.5k kbps to 0.

Here's what I've tried, ANY suggestions would be soooo massively appreciated as it seems like such a weird thing that it's specifically JUST this game.

Used x264 and NVENC, no difference
Lowered my bitrate, no difference
Updated my network card drivers, no difference
Downscaled output to 720p, no difference
Tried game cap, window cap, display cap, all the same
I've ensured my BIOS is set to DOCP and everythings all setup good there
Used MSI afterburner for light overclocking
Checked temps/usage of all devices, GPU hits around 70% at max, everything else is super low, 30% or less.

If you need any information extra to help me out just let me know what you need. I also ran a twitch test for best servers and I've made sure I'm on that server, again it's literally JUST Skyrim this issue occurs on, the game is fine on my end, just the stream turns to poop for a few seconds.

However I've mentioned the various stuff I've tried changing, some of those things aren't in this log as I've been flicking back and forth between settings to try and get some kind of positive result.
