Sound lost during stream

We have been playing with OBS lately for a livestream. Tonight during a stream and recording we lost audio but it continued to show on the meter. So the only way we knew was from comments in live stream. I believe the way I got it back was muted and un muted the audio. We were switching scenes in studio mode a few times but it went out mid scene not during switch. The audio comes from a usb soundboard. Like I said, the meter showed sound but we lost it even in the recording. Any ideas???
