Question / Help Recording is EXTREMELY choppy, but no fps drop?

This machine is way below the system requirements needed to run OBS while also recording a game. You need a discrete GPU.

04:28:52.308: Adapter 0: Intel(R) HD Graphics Family

You might try low resolutions like 480p or 360p and low framerates like 15fps to see if you can eliminate the lag, but at 720p you have a lot of it:

04:35:13.020: Output 'simple_file_output': Number of lagged frames due to rendering lag/stalls: 1759 (22.5%)
04:35:13.021: ==== Recording Stop ================================================
04:35:13.021: Video stopped, number of skipped frames due to encoding lag: 3494/7821 (44.7%)

Also, don't use both of these in the same scene. Pick one.

04:28:53.813: - source: 'Display Capture' (monitor_capture)

While I agree that a discreet GPU is definitely recommended for gaming + recording, that report seems off -- OBS should at least be able to identify which type of intel HD graphics it's running, not just generic "HD Graphics Family".

I would first make sure that your intel graphics drivers are up to date... but also temper your expectations.
