So this is what i have for my recording setup :

1920x1080 @ 60fps
NVENC H.264
CBR 50000
B-frames 0

everything else as default like profile and Preset.

the problem:

whenever I'm recording i look to my preview and everything is very delayed and blurry. it never use to do this before. and when i look back at my recording it doesn't look crisp and very high res. idk what has happened but i am in dire need to get this fixed. the videos look very bad.

Your log doesn't show a recording attempt, so we can't really get any good info. For a guide on recording, read this.

Thank you for posting a log.

That said, I see no recording attempts in said log. That would be useful to have/look at.

Just from your settings/description noted above, I would use VBR@90000kbps (saves on disk space and gives you the overhead needed for complex/high motion scenes), profile=high, level=auto, keyframe interval 4 or 8.

Why aren't you using b-frames? Try setting that to 2.

Usually the error you described is caused by not setting a keyframe value, so it's very important to set that.
