I've been on this for weeks and it's driving me mad. So I got myself an Elgato Chat Link cable and I am able to capture my game audio and chat audio perfectly fine using the Elgato Game Capture HD Software.
As soon as I open OBS though, the Elgato video source only gives me game audio, no chat audio with it. In both cases I'm using use the same source with the same cables and software settings. In case one (Elgato Game Cpature HD suite) it gives me all audio, in case two (OBS with Elgato as a source) chat audio (my voice included) is suddenly missing.
My setup:
P.S. Using the same Elgato feed in vMix gives me game audio + chat audio. There I'm having problems with the video though, so using vMix instead is not a solution. Neither is streaming through the Elgato suite itself, since it's barely customisable and it's colour-keying is terrible.

- PS4 to Turtle Beach Tactical Audio Controller (mixamp) via USB and optical audio
- Headset to mixamp
- mixamp stream out to Elgato analog audio in (analog audio in is ticked in the source properties in Elgato Game Capture HD) via chat link cable
