Question / Help Pixellated video, audio sounds awful, OBS won't record my game audio... please help

I'll preface this by saying I'm totally new to capturing my gameplay.

Somehow I lost the OBS settings that I had before and I don't remember what they were. I've been trying to get everything back to how it was for nearly 3 hours now but everything looks and sounds terrible.

I'm trying to record in Sims 4. These are the issues I'm having:
- pixellated video
- microphone audio sounds terrible no matter what I've tried... I'm using a Blue Snowball Ice
- OBS no longer records my game sounds and game audio

My computer specs:

- Windows 10
- Intel Core i5-7300 HQ CPU @ 2.50 GHz
- NVIDIA GeForce GTX 1050i graphics card

Please let me know if there's any other info I should include. Also these are probably all really stupid questions so I apologize in advance. I'm just desperate for help.
 
You're doing it wrong.

simple output mode
Indistinguishable recording quality
FLV format (not MP4)
Software low cpu use encoder.

Whatever guide you tried to follow is outdated and/or wrong

If I'm still getting a little bit of lag in my video playback when I zoom in/out in game are there any other settings I can change to fix that?
 
You're likely going to need to turn game mode OFF in windows' settings and cap your game's fps.

And make sure your game audio is coming from this audio device.
