It is still an ongoing issue for me right now, many times, i cheered to find the culprit only to find out it s not solely from one thing.
It used to be when using image slideshows as backgrounds, and having multiple VLC media sources in other scenes. I was testing the stutter using a VLC media source 1080p60 fraames with a bitrate around 20K birate but i realized that even that is enough force to make OBS actually stutter.
My first problem was , OBS having a buttery smooth videoplayback on screen on first scene, but start stuttering upon a "scene change". It did not matter if the changed scene was full of stuff or just empty. Returning to the scene with the video test was resulting a periodic stutter.
While trouble shooting the cause of the stutter, i decided to have a single FHD image and added an "imagescroll" effect to it. It was a much better tool for testing the stutter than a video as VLC plugin was being kept out of the numerous posbilities...
Tonight i am doing another stream, this time on 720p60 just to be sure... Tonight might not be the night for i find the cause of the stutter but a step onto the solution.
