Output Failed...

I usually record using the 264x encoder, but I learned its better to use the encoder for your graphics card, in which is AMD AMF H.264/AVC (via FFmpeg)
The moment I start recording, I see this:

I then driver check to see if I had any updates, and didnt. I did a clean reinstall and that didn't help at all
My gpu is an AMD Radeon R5 240 OEM 1GB
The latest Driver version is 21.5.2

I'd like some help with this

Restart OBS and perform a test stream/recording of no less than 30 seconds doing as you normally do, including playing games, full camera or whatever you normally do.
Look in Help menu. Upload the current log file and paste the url to the log in here. Click on the Analize button to start troubleshooting common issues.
