Question / Help Orange/Blue Lines on Captured Video

Problem:
I am trying to stream an Xbox one game through OBS. When I use the Video Capture Device Source, I have vertical blue and orange lines all the way across my screen. I have been googling and searching through the forums for a fix, and all I have been able to find is fixing it on a TV/computer screen itself. (nothing has fixed this) Below is a picture of what it looks like.

I am using a Megewell HDMI Pro-Capture Quad. Any help is appreciated. If there is any other info needed, please let me know and Ill get it on here.

I looked through the links, but I am unable to pick any other option on video format. When I change it to custom, the only option is "any". I can not find anywhere to change it to YVY2. Sorry, I am new at this.

Type your resolution in the 'Resolution' field and then check 'Video Format' field again. Any changes?
