You experience is regular bluetooth behavior, this is no OBS specific issue.

Explanation:
Bluetooth headsets have 2 modes, one high quality mode for music and regular use, and one voip mode for voice conversation. Windows installs one audio device for each mode. At any given time, one device is active and the other is silent, depending on the mode the headset is currently in. The voice communications device has often the words "hands free" in the name.
The communications device is activated whenever an app is activating the headset mic. In this moment, the regular device gets silent and the hands free device gets activated.
If you open OBS, it activates all devices from Settings->Audio. If you configured the headset mic here, OBS activates the headset mic on start, so your headset switches to hands free mode, so your regular audio device gets silent. This is what you described.

To avoid the headset being switched to hands free mode on OBS start, you need to remove the headset mic from Settings->Audio. Also make sure no "default" entry is present - instead, configure each device directly with its name.
