amd radeon rx 480 4gb

You can perform a clean driver installation for your GPU by following the instructions at http://obsproject.com/forum/resources/performing-a-clean-gpu-driver-installation.65/

It seems an Intel driver is being loaded within AMD's encoder.

If this doesn't solve the issue, you may need to disable the built in Intel GPU, or use a different encoder (x264 or QSV).
