Question / Help OBS startup\closeup\during stream crash.

Hey guys, this is going on for a while and i can't figure out what's the issue.
The attached text file contains the log that i get from time to time when it crashes, usually it just crashes with an error but no log.
Any help would be appreciated.

I run it on i73770, GTX660ti, Win7 64bit.
