The fact that you said "Use CBR" and "Enabled CBR padding" leads me to believe that you're using OBS Classic, which is no longer officially supported. You should be using OBS Studio which can be downloaded from the home page, or the download page, or from the GitHub releases page.

Once you have installed OBS Studio, I'd recommend using the auto-config wizard (Tools > Auto-Configuration Wizard (Beta)). The "best" settings vary from system to system, depending on hardware, what you're doing with OBS, and the capacity/reliability of your Internet and network connections. To manage 720p@60fps, Twitch recommends between 3500 and 5000 kbps for your streaming bitrate. Your connection has to be able to handle that amount of bandwidth though. Again, the auto-config wizard should be able to help determine if your system and network can handle 720p@60fps.

Lastly, this should have been posted in one of the Support Forums for Windows, Mac, or Linux. Most of the helpers in the forum check there for threads requiring assistance. I'll see if I can get a mod to move it there.
