This is the first real issue I've had with OBS, really hoping there's a fix. I tried resetting my twitch key twice, as well as changing my stream server. No luck. I've also noticed some frame issues with OBS lately, I've had to stop streaming at 60fps because of this. I don't know if there's a correlation, but maybe there is. Anyways, some help would be much appreciated.

I was about to post here with the exact same issue and tried the same things (reset stream key, changed servers, etc.) I'm thinking this might be an issue on Twitch's end since I streamed fine exactly a week ago with the same settings.

A weird similarity I noticed, both of our text modules "obs-browser.dll" failed to load. Maybe it is a twitch issue but there's been a ton of weird things going on with OBS studio lately. Anniversary update maybe? I'm going to try OBS classic and see how it goes!

EDIT: OK Aegis, I'm 100% with you now. This is a twitch issue, tried on OBS classic and got the "Could not access the specified channel or stream key..." issue. I'll try to contact Twitch on twitter/help center and see if they can do anything.

I've actually just done this, and nothing is different
