OBS recent audio and video desync

I've never had an OBS audio desync issue ever while using desktop source. Only recently have I had issues and I'm not sure why. Sync offset won't work as if I fix the delay in one game it will transfer to another game and I refuse to use game capture because like I said I've never had an issue with desktop source until now. It's not an encoder issue this happens on x264, NVENC h.264 and NVENC AV1. All my drivers are updated and I regularly keep them up to date. I have the latest version of windows 11 24H2. I'm using the latest version of OBS and even reinstalled OBS and it's still happening (I have no plugins installed). I am using multitrack audio for my mic, discord, desktop audio and I have one for Chrome/Music but even with this I never had delay issues ever. SO, what's the problem?

CPU: Intel Core i9 - 14900k
Ram: 32gb DDR5 5200 MT/s
GPU: RTX 4060 Ti 16gb
I have a 1tb ssd and a 4tb harddrive
2 monitors, one is 1080p and the other is 1440p

If I missed anything let me know and I can correct the post

I am very new to OBS and pretty much a noob at this but I was having the same issue when i would go to edit my videos. I had to use a program called Handbreak, it was a free download. When I encode the video using that program it sync's the audio and video. Then I use that file and use it in my video editor. Would be much better if I could skip that step entirely though, so hopefully someone has a simple fix to prevent it from happening in the first place.
