I have a rather complicated situation, hopefully you can help me since I've spent the past 3 days on this and still haven't been able to solve it.
So I currently have the Elgato HD60S capture card.
I am trying to use it to capture the gameplay and desktop audio off of my iMac and send it to my PC where I can then stream it using OBS.
I hooked up the HD60S to my PC and Nintendo Switch and was able to get the audio working perfectly.
When I tried to then replace capture my Nintendo Switch with my iMac that's where I stopped hearing the in game / desktop audio.
The cable I am using to from my iMac to the HD60S is HDMI to USB-C.
The video is coming through perfectly and is loading very nicely in OBS, but I can't get the audio to come through for some reason.
From my research I believe that you have to change the iMac output settings and create a new output so that you can hear the audio from your desktop and send it to the capture card at the same time.
I think I did it correctly:

When I am back on my PC where I am running OBS and have selected Video Capture Device, I get the image but no sound, the little green bar doesn't even move like the other sound options do.
Is there some setting that I am missing in OBS or maybe a setting that I need to change on the actual HD60S to make this work?
