First, update OBS, you're several versions out of date.

Second, run OBS as admin to capture Minecraft. Make sure you install this as well to patch a crash issue with Minecraft: http://nvidia.custhelp.com/app/answers/detail/a_id/4378/~/geforce-hot-fix-driver-version-378.57

I updated to OBS-Studio-17.0.2
EDIT: installing bugfix, trying now
