General things that I noticed in that log:
1. Win10 Game DVR is on (turn it off)
2. Video settings in OBS were 48fps first and later 60 (48 only makes sense on for viewers/streamers with 144Hz monitor)
3. Webcam has unnecessary high resolution (1080p) which makes no sense, when you push out a 720p stream with a webcam picture that takes maybe 3% of the frame, it will also cause higher CPU/GPU load
4. Number of lagged frames due to rendering lag/stalls: 1237 (17.0%)

How to solve rendering lag can be found at point 4.1 and information about problems with gaming monitors is at point 5 here:

This is the same thing that happens to me! And it only happened recently

*FIXED* THE SCREEN LAG / GAME LAG WHEN I HAD OBS OPEN WHICH ALSO FLICKERED WAS DUE TO THE LATEST UPDATE ON OBS. I UNINSTALLED THIS VERSION AND THEN FOUND PREVIOUS VERSIONS ONLINE - CHECK HERE https://github.com/jp9000/obs-studio/releases - I DOWNLOADED 20.1.1 BUT IM SURE YOU COULD USE 20.1.2 IF YOU WANTED.

TRY THIS BEFORE TAMPERING WITH YOUR COMPUTER! ALSO MAKE A NOTE OF THE PREVIOUS SCENES AND SOURCES YOU HAD BECAUSE I ACCIDENTALLY DELETED MINE WHEN I WENT BACK TO THE OLD VERSION!

I am another one who is having the same problem since yesterday. Did a stream on thrusday with no problems at all, but yesterday my fps was dropping to 19-21 fps. Then I realise that if i clicked on another window the FPS fixed on 30fps.

I'm also attaching my log from yesterday.

@MiseryCop try downloaded previous versions here: https://github.com/jp9000/obs-studio/releases

1) uninstall current OBS: Go settings > Storage > This PC (C:) > Apps & Games > Type OBS and uninstall
2) Download earlier version of OBS (https://github.com/jp9000/obs-studio/releases) preferably 20.1.1 or you can use 20.1.2.

My specs are AMD R9 390, 4690K, 8GB Ram. This issue occurred around the latest update (a couple of weeks ago?) so I decided to try this. Let me know if it works for you!

So, I also did what Kyak recomended and the problem persists. BUT, I tried another solutions and I found a temporary one: turn the VSYNC ON in-game!

The link that BK-Morpheus sent is pretty useful:

So, three games that I was having difficulties, Elite: Dangerous, The Witcher 3 and Northgard, none I was using VSYNC. I didn't have these kind of problems before, so I never mind with these. And then yesterday I turned the VSYNC ON and, finally, my OBS fps stucked at 30.

During my tests I had only one moment with Witcher 3 that the fps started to drop again (about 30s), when I called a horse and the fps in game also dropped a little. So, because of that, I'm not trusting that I will always have a smooth stream from now on. I don't know what changed from thursday to friday, maybe a windows 10 update or maybe this is a bad sign from my GPU, but I really didn't have any of theses problems before.

Hello, I'm pretty new to OBS, and I am noticing that my game captures turn out really chappy/laggy although my game runs at around 200 fps. I do not know what to do. I have noticed that many people put their logs in to see if there is something to be fixed...
Before I restarted my PC around 2 months ago, I had OBS and there were no problems, so I thought it was a new version or something. So I downloaded an older version, but the same thing happened. If someone could help me out here, that would be wonderful. Thank you.

16:42:04.840: Windows Version: 10.0 Build 17134 (revision: 523; 64-bit)

Windows is out of date and game mode is on, which negatively impacts OBS performance. Run the October 2018 update, and turn game mode off.

Do that, then do a session where you notice the problem and upload that log. Start recording or streaming, observe your issue, stop the session, then upload.
