I've installed OBS 27.2.4 along with BlackHole 2ch. Also I configured Multi-Output to use Built-in-Output and BlackHole 2ch. In OBS, I created an Audio Output capture with BlackHole 2ch as the selected device and finally in the mac System Preferences for sound, I selected Multi-Output device for sound output. I am able to view the video in OBS, However there's no sound. On the other hand, when I launch iTunes, I am able to select and play music via the mac internal speakers without any issues without changing any settings. This is on a MacBook pro running macOS Catalina 10.15.7
