Question / Help OBS 23.2.1

Started up 16.5 after not using for months, new OBS 23.2.1 available, 2 stupid questions, do I uninstall 16.5 1st or does 23 overlay into it? Do I/should I also include plug-ins and realsense source?

Just install the newest over it. You probably don't need the realsense source.

Yes. It's possible some may break since that version is so old, but just updating OBS does not delete user data (profiles and scene collections).

Just in case I listed all my settings. I was trying to record Splinter Cell games and for some reason OBS and the game didn't like each other, So I've been using GeForce Shadow play something, something but prefer OBS.

P.S. I just loaded the 23.2.1 and notice a (new) Nvenc h.264 I should use it instead of the older Nvenc H.264 yeah!

It will be more efficient. I don't think it provides higher quality unless you've using one of the newest Nvidia chips, but as long as it works well for you, you should feel free to use it.
