I typically use Windows with a USB3HDCAP so my knowledge of this subject is unfortunately limited.
My colleague uses OBS on a Mac with a Blackmagic Intensity Thunderbolt. The only camera he's been able to use successfully is a borrowed Sony EX-1R. Unfortunately he has to return this camera and buy his own. He said he tried every camera at three camera stores (!) yesterday without success.
He has the latest version of the BMD driver and OBS.
He tells me:
If anything jumps out at anyone here as something we might be doing wrong, we would appreciate knowing about it.
I'm sure he's checked the basics like replacing the HDMI cable, but I have just asked him to double check that.

I've noticed though that with OBS it often doesn't detect the camera right away and needs to have the settings changed 4 or 5 times and then it suddenly randomly starts working on the 5th time changing it to 720p 59.9 fps. Have you had any similar issues or have any suggestions for what to try?
BMD desktop video is detecting the shuttle, but the software seems to only be there for changing settings, it doesn't display the video signal. So, I open up BMD desktop video, plug in the shuttle, it shows up and tells me the video input is HDMI and video output is 720p59.94, and then as soon as I launch OBS, the BMD desktop video goes from displaying "720p59.94" to just a dash. It's like it stops outputting a signal as soon as OBS is running.
