NDI Source name missing

I have installed the NDI Plugin using the windows installer. When I go to add, I have an option to add NDI Input. When I click to add, the drop-down list for Source Name is blank. The PTZOptics NDI-30x camera is not seen. I have also installed the PTZOptics toolbar and I can make the camera move with it, but I cannot access the image.

I am running the camera on a ubiquiti unifi network.
